J.D. and the rest of the interns begin their first day at Sacred Heart. J.D. does his first night on-call, and realizes that Dr. Cox is much nicer and more helpful than Dr. Kelso. J.D. is attracted to fellow intern Elliot.
Elliot takes issue with the endearments Dr. Kelso uses on her, but naively follows Dr. Cox's advice and confronts him over it. J.D. misses Turk's company with how busy both of them are, and he worries that a post-op reaction with a patient is Turk's fault. Elliot and J.D. are getting close, but an interruption might make the difference between being friends and something more.
The three interns have to face death for the first time: J.D.'s patient is a headstrong but kindly old woman, Turk's is a young man who just needs some company, and Elliot's is a Hispanic woman who speaks no English.
J.D. has to decide between viewing Dr. Cox or Dr. Kelso as a mentor. The decision isn't made any easier when all three, along with Dr. Kelso's lapdog Dr. Steadman, play a game of golf together. Cox is supposed to write J.D.'s intern evaluation but tells J.D. to do it himself. When J.D. refuses, Dr. Cox reveals that he wanted J.D. to do it so that he could discover his own shortcomings and judge himself.

When drugs go missing, Elliot suspects a patient who is a former drug addict, but Alex defends him. Elliot tells J.D. that she thinks Alex will hurt him and J.D. accuses her of being jealous. Alex ends up being the one who took the drugs. She and J.D. part on uncertain terms, and J.D. tries to confront Elliot about her accusation of the patient. The two finally make up when J.D. apologizes.

In the aftermath of Jordan's revelations, everyone is estranged from each other. After Jordan revealed that she slept with J.D., he expects the worst from Dr. Cox. Elliot, embarrassed by Jordan's revelation about her lingering feelings for J.D., is avoiding J.D. at all costs. Guest-stars Colin Hay as the troubadour.
The residents spend a night on-call with no attendings available, so they have to deal with the responsibility of running the hospital on their own, struggling to make a tough decision about a patient. Meanwhile, Cox deals with his feelings for Jordan.
Dr. Kelso starts an impromptu competition with the residents, offering an all-expenses-paid medical conference outing in Reno if they can bring him the most interesting case study: J.D. is torn to compete and to keep in Dr. Cox's favor by not doing so. Another competition begins when the entire hospital takes advantage of Dr Kelso's annual good mood the next day. Elliot worries that she isn't getting on well with Carla.

Not wanting to repeat their last disastrous relationship, J.D. and Elliot agree to become sex buddies to limit their feelings for each other, although this doesn't entirely work out as they planned. Jordan and Dr. Cox worry about a slight cough their son has and try to get him seen by the hospital's own pediatrician, who is just as obnoxious as Dr. Cox is. The residents get their own interns: Turk is too firm, J.D. isn't firm enough, and Elliot is a mix of both, especially when Dr. Kelso compares the clumsiness of one of them to her own. Guest-stars Christopher Meloni as Dr. Dave Norris.
J.D. distracts the janitor and Troy with a riddle. Dr. Cox and Dr. Steadman compete for the same promotion for the residency director position. Sean and Elliot's relationship hits rocky ground, and they virtually break up; J.D. grabs this opportunity to try to tell her of his growing hidden feelings for her.

When Danni has plans for her and J.D. to move into a place together, J.D. immediately realises he needs to break up with her - only he's never actually dumped anyone before. The janitor starts flirting with Elliot again. Dr. Cox realises that by losing his fearful influence over his residents, he may have lost control over them.

Dr. Casey and J.D discover the janitor's toilet on the roof-top (dubbed the Epiphany toilet); it warrants the name after many of the staff realise solutions to the problems they're having after using it. Dr. Kelso closes down an entire wing of the hospital to save money: Dr. Cox and Carla fight back by using his own office as a room. Turk asks J.D. to be his best man, but he realises later he wasn't Turk's first choice. Elliot seeks help from Dr. Casey as she is becoming fearful to intubate patients after botching an intubation.
Jordan, Danni and Ben return to help prepare a gathering for Dr. Cox's son's first birthday. Turk and Carla clash over removing Turk's mole, and changing Carla's last name after they marry, while one of Ted's band members quits. Dr. Cox angrily blames J.D. for a patient's death and dismisses him, but Ben calms Dr. Cox down and talks some sense into him, and he eventually forgives J.D. In the end it is revealed that Ben was the patient that had passed away and Dr. Cox was lingering over a memory until he was able to forgive himself. Guest-stars Brendan Fraser as Ben Sullivan.
Dr. Cox is struggling to recover over Ben's death, and doesn't seem to appreciate J.D.'s efforts to help his situation; this is exacerbated by Jordan's self-absorbed friends staying in his apartment. Carla attempts to teach Todd, Dr. Kelso and Dr. Cox about sexual harassment, and a new female attending surgeon, Dr. Miller, puts the head of everyone in the OR in a spin.
The wings of a butterfly can influence even random chances: this episode revolves around two alternate storylines, sharing the common influences of a butterfly's landing on cleavage, Turk's lucky do-rag (head-scarf), a Snickers bar Carla sits on, a stuffed pink dog that belongs to Elliot's young patient, the janitors' custodial closet, and another patient's heart problems. Also, a flashback from the janitor reveals how he decided to become one.

The janitor plays an elaborate ruse on J.D. and Turk about having a twin brother.

A later distraction with Dr. Miller during surgery causes nerve damage to a young concert pianist, to whom he can't admit his mistake. J.D. covers Elliot's stint as a hospital clown, only to have the janitor as a sidekick. A patient's advice to Elliot makes her realise just how good a friend J.D. has been: her feelings take charge and they end up sleeping with each other again; but a surprise visit by Sean afterwards has her leaving with him.
